LM5050-1: reverse voltage protection schematic using LM5050-1

Hi, 

 i need to provide a schematic for Input reverse voltage protection using LM5050-1. I ordered AN-2087 LM5050-1EVAL Evaluation Board. and now i noticed the schematic of the eval board is not the same as schematic of reverse voltage protection schematic suggested by LM5050-1 datasheet. Please let me know how i can modify the eval board to have voltage protection feature?

in the second one are there two different Ground? 

how i can add the D3 to the first schematic?

can the eval board work as a input voltage protection without adding the D3?
