BQ33100 Shutdown Mode

I am using the BQ33100 in a design almost identical to the BQ331000EVM. I cannot get it to come out of shutdown mode, if I apply power to the VCCPACK pin it come out of SHUTDOWN MODE while the power is applied to the VCCPACK pin but goes back into SHUTDOWN MODE as soon as the voltage is removed.
